Please complete this request form entirely. Any missing information could result in a delayed or denied request. 
 

If you are not the original purchaser, please provide the serial number. In most cases, this will be located above the door. You may also provide original purchaser's name if known.

Please provide pictures of the area in need of repair. Multiple pictures from multiple angles (close up to show the problem area and farther back to identify location of problem.) These may be submitted thru this form, by texting them to 270-789-4236 or emailing to claims@backyardworld.net. If pictures submitted via text or email, please include your name and contact info in the email or text. Failure to do so will impact response time.

This form may be printed and mailed (along with any applicable documents and required pictures) to Backyard World attn Claims, PO Box 344, Campbellsville, KY 42719.

Response time for completed submissions will typically occur within 1-2 business days.

Warranty claims for all PRE-OWNED products will be denied, unless noted otherwise on signed invoice.
